How to Prepare for Your Evaluation

Complete the Questionnaire

In order to provide the best evaluation possible, we have assembled a patient history questionnaire, as well as registration and consent forms for your review and completion through our secure patient portal. The information you provide will be used by the Neuropsychologist during the interview to better focus their time and attention on your specific concerns. Therefore, if you do not thoroughly and accurately complete the questionnaire and return it at least one week prior to your appointment, you may be charged a noncompliance fee of $500 for which insurance will not be billed.

The completed questionnaire must be returned no later than 1 week prior to the appointment or it may have to be rescheduled.

Keep the Appointment

Don’t change the appointment without proper notice. We complete a courtesy reminder call and/or text message 1 week in advance of each appointment. You are responsible for confirming your appointment by respond to prompts during the call and/or via text. This MUST occur within 48 business hours of your scheduled appointment time, or your appointment will be cancelled. If you are cancelling your appointment, you MUST do so at least 48 business hours in advance, directly with a staff member during business hours, otherwise you will be charged a “No Show” fee of $500.00. For feedback appointments, the appointment MUST be cancelled with at least 48 business hours’ notice, or you will be charged a “No Show” fee of $50.00. Insurance will not be billed for “No Show” fees.

The Day of the Appointment

  • Arrive 30 minutes BEFORE your scheduled appointment. Arriving more than 10 minutes late for your appointment could be considered the basis for the appointment to be treated as a no show and for fees to be assessed.

  • Bring a copy of your insurance card and photo ID. As a courtesy, we will bill your insurance if you provide accurate proof of coverage at the time of service. You are expected to pay any/all deductibles and co-pays at the time of service. You are responsible for paying any balance that is not covered by your insurance.

  • A parent or legal guardian with proof of guardianship must accompany all children under the age of 18 years of age. No child is to be left in the waiting room without adult supervision. In order to be able to participate fully in the evaluation process, parents are asked to only bring the child who is to be seen to the appointment. If you are unable to make prior arrangements for any additional children, please call our office to reschedule the appointment no later than 48 business hours prior to the appointment.

  • Bring a someone who is familiar with your medical history and current issues.If you, or the patient, are non-verbal or a poor historian, unable to provide accurate information regarding the patient’s situation, then the patient MUST be accompanied by an adult who is a reliable historian, familiar with their history and current issues when meeting with the neuropsychologist.

  • Get plenty of sleep the night before the appointment. If you are not well-rested for your evaluation appointment, this will negatively affect your test performance and make it more difficult for the Neuropsychologist to determine an accurate diagnosis. Therefore, if traveling from outside the Anchorage bowl area for a morning appointment, please find accommodations for the evening prior to your evaluation.

  • Bring any hearing aids, contact lenses or glasses, if you have them.

  • Eat before the appointment and bring a snack. We have snacks and beverages available for patients but if they have food allergies or dietary restrictions we may not be able to accommodate that.

  • Submit a list of your current medications (and doses) with the consent forms you will need to return prior to the evaluation. You should also bring the medication list with you to the appointment.

  • Do not take any sedating medications or ADHD medications the morning of your appointment. Bring those medications with you to the appointment.

  • Submit any medical or school records you have (past and present) related to your problem.
